Before your Exam

Forms on this page can be used by patients who would like to have their forms filled out before they arrive for their appointment. Please print and complete the forms and bring them with you to your appointment.

Insurance Coverage & Imaging Authorizations
We are happy to accept most major insurances and many other carriers as well. We work with many workers’ comp companies as well as attorney lien providers in an effort to be accessible to more people in our community. After Your Exam
Getting your Results
After your exam is concluded, we will send your referring physician the report immediately once it is reviews by our radiologists.

Sharing Feedback
We welcome any feedback you can provide about your experience in our centers. You should receive a survey via email or direct mail within the next week that will allow you to share your feedback and/or suggestions. We utilize these surveys as a means to improve our overall service so we greatly appreciate your time and any information provided.
